Cardiovascular Program

The Cardiovascular Program at Suburban Home Health care includes a full assessment of all body systems with a primary emphasis on a patients cardiovascular system. Our skilled nurses affiliated with this program employ specific Suburban Home Health Care CHF and HTN clinical guideline protocols.

Examples of Cardiovascular  processes we manage, include but are not limited to:

  • HTN
  • Dysrhythmias
  • Anemia
  • Heart Failure
  • s/p myocardial infarction
  • Angina
  • PVD
  • CAD

Currently we offer interventions/teaching in the following areas:

  • CV medication management.
  • Parameters for blood pressure ranges and MD reportable values.
  • Diet modification.
  • Venipuncture and coumadin management.
  • Adverse effects assessment to medication therapy.
  • S/S of disease processes and reporting to RN/MD.

The primary goal of our Cardiac Care program is to foster the independence of a client and eventually allow for the client to independently manage their cardiovascular status.
