Community Liaison
We are proud to offer availability of an on call nurse liaison. The nurse liaison facilitates the discharge of patients needing Home Health Care services. They consult with the hospital staff and physician regarding treatment plans, insurance coverage, and arrange for a smooth transition from the hospital or nursing facility to the patients home setting. Assessments received from within the Boston area will be done in 4 hours.
Information we will need:
- Name of facility and address
- Name of caller and title
- Name of patient first and last
- Location of patient floor & unit
