Methadone @ Home Program

Patients who qualify for our Methadone Program must have the following items prior to admission to our agency:

  • letter of medical necessity signed by the patients MD
  • last dose letter from the hospital
  • patient must already be established with a methadone clinic
  • hospital DC paperwork

Important Insurance/Eligibility Information:

  • Medicare will not pay for daily visits to administer methadone. The patient must exhibit an acute underlying medical condition which requires daily nursing services (ie. wound care, diabetes, exacerbation of COPD, recent hospitalization)
  • Medicaid Basic does not cover home dosing. Patients with this insurance are accepted on a case by case basis only, please call to speak with a clinical manager, 617-264-7100.
  • Neighborhood Health Plan (N.H.P.) requires prior authorization for methadone administration.
  • Medicaid w/PCC plan, will require prior PCC authorization before we can begin visits. Medicaid patients are not necessarily required to be homebound, but may require daily dosing at home due to their frail condition, compromised immune system, and or need to limit exposure to the public.
