Suburban Home Health Care currently employs an Advanced Care Team- A.C.T. Members of the A.C.T. team are specially trained nurses, skilled in assessing and providing wound care by utilizing current treatment modalities. Our A.C.T. team provides quality wound care based on the most recent scientific knowledge regarding wound healing.
Components of the scientific rationale:
- Maintain a moist wound environment
- Control exudates
- Provide an effective barrier to microorganisms
- Require infrequent dressing changes
- Provide thermal insulation
- Obliterate dead space
- Aid in removal of devitalized tissue

- Pre-discharge or pre-referral assessment. (This assessment is free of charge.)
- CWCN Consultation, as needed
- Treatment formulary specifically developed for Suburban Home Health Care
- Specific treatment modalities developed for all types of wounds including wound V.A.C therapy
